We accept many insurance plans. If you do not see your plan listed below, please call our office to check with our staff. If we do not accept your insurance, you may still be seen and we can provide you with the documentation you will need to submit a request for reimbursement to your insurance company or to your Flex account. For those patients without insurance, we do offer a discounted self-pay rate and the lab we use provides their services at a significantly reduced rate. Prior to your appointment, please complete the office forms below and bring them with you to your appointment. If you choose not to use the forms on this site, please arrive 20 minutes in advance of your appointment time to complete them in the office; bring your insurance card and a photo ID.
